Complete this form if you wish to submit an appeal after the outcomes are available.
Appeals must be submitted to the school principal no later than Monday 5 November 2012. The principal will complete the school's section and return the form to the Manager Selective High School and Opportunity Class Placement no later than Monday 12 November 2012.
Complete this form if claiming an illness or misadventure affects the student's performance in the test or at school. All claims of illness at the time of the test must be submitted at this time. Illness/Misadventure claims must be submitted by Friday 10 August 2012.
Complete this form if the student is not returning to NSW to sit the Opportunity Class Placement Test.
Complete this form if you wish to receive further details of your child's score. Requests are processed at the end of each month after outcomes are available.
