Appeal form (PDF)
Complete this form if you wish to submit an appeal after the outcomes are available.
Appeals must be submitted to the school principal by 20 July 2012. Principals add comments and return the form by 27 July 2012.

Formal exemption (PDF)
Complete this form if the application is not being submitted through a NSW non-government school. Not required when the application is submitted through the school or for interstate, overseas or home-schooled applicants.

Illness/misadventure (PDF)
Complete this form if claiming an illness or misadventure affects the student's performance in the test or at school. All claims of illness at the time of the test must be submitted at this time. Illness/misadventure claims must be submitted by 5 April 2012.

Report of academic merit: interstate and overseas applicant (PDF)
Complete this form if the student is not returning to NSW to sit the Selective High School Placement Test.

Report of academic merit: virtual selective class provision, xsel (PDF)
Use this form if applying for xsel, the virtual selective class provision. Print the form and ask the principal to complete and send with application.

Request for scores (PDF)
Complete this form if you wish to receive further details of your child's score. Requests are processed at the end of each month after outcomes.
